What is a Primary Care Network?

A Primary Care Network (PCN) is a group of GP surgeries working together in to focus on local patient care. By working as part of a PCN, your GP surgery can offer you more services by sharing resources and employing more staff.

PCNs build on existing primary care services and enable greater provision of proactive, coordinated and more integrated health and social care for people close to home.

GP practices are working together with community, mental health, social care, pharmacy, hospital and voluntary services in their local areas in groups of practices known as primary care networks (PCNs).

Great West Road Primary Care Network:

At Great West Road, we are a family of 8 practices and an inclusive group of healthcare professionals that work collaboratively to provide high quality healthcare for our patients. We care, support and empower our patients with wider and innovative services.
